Secondary Transition Services
Following the annual PPT meeting after a student’s fifteenth birthday (or earlier when appropriate) the plans for transitioning into adult life will be discussed. “Transition services means a coordinated set of activities for a student, designed within a results-oriented process, which facilitates movement from school to post-school activities, including postsecondary education, vocational education, integrated employment, continuing and adult education, adult services, independent living, or community participation.” (34 CFR § 300.43 (a))
